Program Details
Eligibility: Physician referral for diabetes self-management education and individual consults.
Fees: Diabetes self-management education is covered by Medicaid and Medicare and most insurance plans. Call insurance company to verify coverage.

Program Description:
Provides inpatient diabetes management, outpatient diabetes self-management education, and individual consults. ADA-recognized for self-management education. 8-hour classes offered daytime (all day or 2 half-day sessions); Saturday classes also available. Covered by most insurance, including Medicaid and Medicare. No charge for 1.5 hour follow-ups at 3, 6, and 12 months to review progress. Program also available at WakeMed Cary Hospital. Pre-diabetes classes are available at WakeMed Cary, but these classes are not covered by insurance. Please call 350-7292 for information and scheduling at both locations.
