Program Description:
Agency providing in-home occupational and physical therapy services aiding in evaluation, treatment and prevention of falls. Also provide rehab services for other common disabilities associated with aging. Goal is to maximize safe mobility in the home, avoid hospital/emergency visits, and avoid nursing home placement. Medicare provider, accepts other insurance as well.
