Eligibility: New patients must have a complete physical to become established.
Fees: $69 for most appointments in office, and low costs for lab work. Housecalls available for established medical home patients for a fee of $68 plus mileage, depending on distance.

Program Description:
Physician practice offering both office and home visits on a private pay basis. Does not contract with or bill insurance, including Medicare or Medicaid. Patients may file for reimbursement by their private insurance company, but coverage is not guaranteed. Medicare and Medicaid claims cannot be filed or submitted for reimbursement. For $549 per year, patients can have office visits for only $20 including most common lab work. With this plan, housecalls are only $68 each, plus $.20/mile for destinations over 5 miles. This practice sees a lot of Medicare and some Medicaid patients, but they must pay fees as above out-of-pocket.
