Eligibility: Patients must be between age 18 and 64, and below 185% of the federal poverty level (approx. $22,980 annually for one person or $31,020 for two).

Program Description:
Shepherds Care Medical Clinic provides care for individuals between 18 and 65, with chronic diseases such as asthma, diabetes, high blood pressure and more. Patients must be between age 18 and 64, income must be below 185% of the federal poverty level (approx. $22,980 annually for one person or $31,020 for two), and reside in Eastern Wake County or portions of Nash, Franklin and Johnston counties that adjoin Eastern Wake County.
