Hours: Monday-Friday, 9:00AM - 5:00PM
Eligibility: No limitations, other than ability to pay for services.

Program Description:
Provides Registered Nurse care coordinators who manage the care of seniors dealing with complex medical situations. Make weekly home and facility visits, manage medications, accompany seniors to doctor appointments, and communicate with family members. Mission is to prevent hospitalizations, medication errors and visits to the emergency room.
